About The Position
The Hospice Admission RN provides skilled nursing services to patients in their homes under a physician’s plan of care. The role involves working in community care facilities and homes in the Norway/Bridgton area, with an autonomous, patient-focused approach and flexible scheduling options. The nurse collaborates with patients and families to develop realistic, measurable care outcomes, supports informed decision-making, and addresses patients’ holistic health care needs, including pain control and symptom management.
Requirements
- Current Registered Nurse (R.N.) license in the State of Maine.
- Current CPR card.
- One year of recent clinical nursing experience.
- Reliable car with insurance.
Nice To Haves
- ANA certification in Home Health or Community Health Nursing is encouraged.
Responsibilities
- Provide skilled nursing services to patients in their homes under a physician’s plan of care.
- Develop plans of care with realistic and measurable outcomes in collaboration with the patient and family.
- Provide education and promote patients’ rights and responsibilities to support informed decision-making in the plan of care.
- Assess and identify patients’ holistic health care needs.
- Demonstrate competency in assessment and technical skills.
- Apply knowledge of pain control and symptom management.
