About The Position
Central Coast Home Health and Hospice is seeking compassionate and motivated Registered Nurses to join their team in multiple territories across San Luis Obispo County. Opportunities are available in Home Health, Palliative Care, or Hospice settings. The Home Health RN provides patient-centered care in patients' homes, collaborating with an interdisciplinary team, and offers autonomy and flexibility. The Palliative Care RN cares for patients with life-limiting diseases, focusing on symptom management, end-of-life decisions, and education, working collaboratively with a multidisciplinary team. The Hospice RN plans, organizes, and directs hospice care, building community resources to meet patient and family needs, and promoting patient and family choice, continuity of care, and quality of life for dying patients.
Requirements
- Current California Registered Nurse (RN) license
- Minimum one year of professional nursing experience
- Current CPR certification (Healthcare Provider)
- Valid driver’s license with clean driving record
- Reliable transportation and current auto insurance
- Ability to work independently and manage a flexible caseload
Nice To Haves
- Home Health/Palliative Care/Hospice experience preferred but not required
Responsibilities
- Deliver skilled nursing care in patients’ homes
- Assess patient needs and develop individualized care plans
- Coordinate care with physicians and interdisciplinary team members
- Complete timely and accurate clinical documentation
- Educate patients and caregivers regarding treatment plans and health management
- Promote continuity of care and positive patient outcomes
