About The Position

Benchmark is seeking a compassionate Registered Nurse to join the team at Putnam Farms. The role delivers nursing care to residents in a warm, comfortable, home-like environment and works alongside a supportive team focused on building community and lasting relationships with associates and residents. The position is listed as full-time evening work from 3 p.m. to 11 p.m., with a stated pay range of $35–$39 per hour. Requirements

• Current state license as an LPN and CPR certification.
• Graduate of an approved LPN program, as required by the state.
• One to two years of experience working as a nurse is preferred.
• Previous experience working with the elderly in a paid or volunteer position is highly desirable.

Responsibilities

• Accurately assess, communicate, and document residents’ status.
• Observe and report significant changes in residents’ behavior or health to the Resident Care Director, physician, family, Resident Care Assistants, and Executive Director, in accordance with state regulations.
• Direct and supervise the daily work assignments of Resident Care Assistants.
• Mentor, guide, and direct Lead Resident Care Assistants in all aspects of their jobs.
• Coordinate care needs with community providers through an effective case management process.
• Balance service demands with support for resident independence.
• Recognize and assist in preventing elder abuse, neglect, and exploitation, including financial exploitation, and report concerns to appropriate sources as required by state regulations.
• Respond to emergencies and personal emergency response system alerts, and know when to call for backup.
