About The Position

Wilson Health is seeking a direct hire Home Health & Hospice Registered Nurse for its location in the Sidney, Ohio (North Dayton, Ohio) area. This role is responsible for the clinical care of the client in their place of residence, utilizing the nursing process and following established policies and procedures in support of the medical plan of care. The position involves coordinating case management and documentation progress of the client. The RN Case Manager will work in a fast-paced environment that demands flexibility, critical thinking, and a patient-centered approach. The role may require occasional travel to healthcare facilities and patient homes within a 6-county radius of Auglaize, Champaign, Darke, Logan, Miami, and Shelby Counties.

Requirements

• Current and valid Registered Nurse (RN) license.
• Minimum of 2 years nursing experience, preferably in case management or related field.
• Strong knowledge of healthcare systems, insurance processes, and patient advocacy.
• Excellent communication, organizational, and problem-solving skills.
• Ability to work independently and as part of a multidisciplinary team.
• Proficiency with electronic medical records (EMR) and case management software is a plus.

Nice To Haves

• Previous Home Health/Hospice experience preferred.
• Medical/Surgical and Long Term Care experience helpful.

Responsibilities

• Use the nursing process and perform initial holistic nursing assessment of the client.
• Establish short and long term goals, provide skilled nursing service in the home.
• Document Oasis accurately and completely.
• Complete necessary forms required for payer billing.
• Assess safety of the patient environment.
• Develop a comprehensive plan of care based on nursing assessment and oasis.
• Complete all referral forms to appropriate referrals.
• Review, teach, and administer medications in a safe and timely manner.
• Recognize and utilize opportunities for health teaching and provide patient education.
