About The Position

Elara Caring is seeking a compassionate PRN Home Health Registered Nurse to provide skilled nursing care to patients in their homes. The role is intended for an experienced home health RN who can work independently, manage visits efficiently, and provide meaningful one-on-one patient care. It offers flexible, field-based autonomy and a consistent part-time schedule while supporting patients in recovering, managing their health, and maintaining independence at home.

Requirements

• Current, unrestricted Texas RN license.
• Previous home health nursing experience.
• Ability to work independently and manage visits efficiently.
• Availability to commit to a consistent part-time schedule.
• Valid driver’s license, reliable transportation, and current auto insurance.
• Willingness to travel throughout the territory.
• Ability to sit, stand, bend, lift, and move intermittently, and lift 50–100 pounds.

Responsibilities

• Complete admissions, assessments, routine visits, recertifications, and discharges.
• Develop and update individualized plans of care.
• Monitor patients’ conditions and communicate significant changes.
• Coordinate care with physicians, caregivers, and the interdisciplinary team.
• Educate patients and families about medications, conditions, safety, and self-care.
• Complete accurate and timely clinical documentation.
• Help prevent avoidable hospitalizations through proactive nursing care.
