About The Position

Signature HealthCARE of Norfolk is seeking a Registered Nurse to provide resident-centered nursing care in a 169-bed facility offering short-term rehabilitation and long-term care. The RN provides direct clinical care, supports residents’ comfort, dignity, and safety, and works with an interdisciplinary team to develop and review individualized care plans. The role also includes supervising nursing assistants and medication aides during the assigned shift, completing clinical documentation, coordinating resident transitions, and contributing to quality and staff education activities. The position is full-time, with a schedule of 3 p.m. to 11 p.m.

Requirements

Current, active RN license in the state of practice.
Minimum of one year of clinical nursing experience.
Strong clinical observation, medication administration, and documentation skills.
Knowledge of federal and state long-term care regulations.
Effective verbal and written communication skills.
Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
Familiarity with electronic health record systems.
Proficiency with Microsoft Office applications.

Nice To Haves

CPR/BLS certification, preferably from the American Heart Association or American Red Cross.
Long-term care experience.
Experience in a supervisory or charge nurse role.

Responsibilities

Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
Assist residents with activities of daily living and ensure their comfort, dignity, and safety.
Supervise and direct nursing assistants and medication aides during the assigned shift, in accordance with facility policies and regulations as applicable.
Monitor residents’ physical and emotional status and notify the attending physician and responsible party of changes in condition.
Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
Complete accurate and timely clinical documentation according to facility policies and procedures.
Coordinate resident admissions, transfers, and discharges according to established procedures.
Participate in quality assurance activities, staff education, and regulatory surveys as needed.
Perform other duties as assigned by the Director of Nursing or designee.
