About The Position

Amedisys is hiring a full-time, day-shift Registered Nurse case manager to provide home health care. The role focuses on helping patients heal at home through assessments, care planning, patient education, and coordination with the care team. The position includes on-call responsibilities and supervision of LPNs and home health aides. Compensation is $77,000–$96,000 annually, with a $7,500 sign-on bonus and mileage reimbursement.

Requirements

• At least one year of clinical experience as a Registered Nurse; candidates with less than one year require approval by the VP of Clinical.
• Current RN license specific to the state(s) where the employee is assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.
• Candidates with less than six months of RN clinical experience must participate in the RN Intern program.

Responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
• Refer patients to other disciplines when indicated by their needs, or document the rationale for not making a referral.
• Promote patient health and independence through teaching and appropriate rehabilitative measures, including instruction in self-care techniques.
• Supervise LPNs and home health aides.
• Complete documentation accurately and on time.
• Regularly communicate patient progress to the clinical manager and care team.
• Plan and provide staff education.
• Perform on-call responsibilities and provide on-call services to patients and families as assigned.
• Participate in clinical development and continuing education programs.
• Perform other duties as assigned.
