About The Position

Amedisys is hiring a full-time registered nurse case manager to provide home health care. The role focuses on helping patients heal at home through assessment, care planning, education, and coordination with the care team. The position includes on-call duties and supervision of LPNs and home health aides. Compensation is $72,000–$90,000 annually, with a $7,500 sign-on bonus and mileage reimbursement.

Requirements

• At least one year of clinical experience as a registered nurse; candidates with less than one year require approval by the VP Clinical.
• Current RN license specific to the state or states assigned.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.
• Candidates with less than six months of clinical RN experience must participate in the RN Intern program.

Responsibilities

• Assess patients and collaborate with the care team to develop and implement plans of care.
• Refer patients to other disciplines when indicated by identified needs, or document the rationale for not doing so.
• Promote patient health and independence through teaching and appropriate rehabilitative measures, including instruction in self-care techniques.
• Supervise LPNs and HHAs.
• Complete documentation accurately and on time.
• Communicate patient progress regularly to the clinical manager and care team.
• Plan and provide staff education.
• Perform assigned on-call responsibilities and provide on-call services to patients and families.
• Participate in clinical development and continuing education programs.
• Perform other duties as assigned.
