About The Position

Amedisys is hiring a full-time, day-shift Registered Nurse case manager for home health. The nurse helps patients heal at home by assessing their needs, coordinating and implementing care, promoting health and independence, and working with the care team. The position includes on-call responsibilities and offers a $7,500 sign-on bonus. The stated salary range is $80,000–$100,000; compensation may vary based on geographic market, job-related knowledge, skills, and experience.

Requirements

• At least one year of clinical experience as a Registered Nurse. Candidates with less than one year require approval by the VP Clinical.
• Current RN license specific to the state or states assigned.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.
• Candidates with less than six months of RN clinical experience must participate in the RN Intern program.

Responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
• Refer patients to other disciplines as indicated by their needs, or document the rationale for not doing so.
• Promote patient health and independence through teaching and appropriate rehabilitative measures, including helping patients learn self-care techniques.
• Supervise LPNs and HHAs.
• Complete documentation accurately and on time.
• Regularly communicate patient progress to the clinical manager and care team.
• Plan and provide staff education.
• Perform on-call responsibilities and provide on-call services to patients and families as assigned.
• Participate in clinical development and continuing education programs.
• Perform other duties as assigned.
