About The Position

Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services. In this role, the nurse guides patients and families through the care continuum by assessing needs, coordinating care, supporting discharge and transition planning, and managing ongoing outpatient care. Under the supervision of an RN or MD, the nurse helps ensure patients receive the appropriate level of care, provides education, coaching, and treatment-decision support, and collaborates with a Care Advocate and Social Worker. The position is part-time and weekend shift. Compensation includes per-visit-point pay for visits and hourly pay for certain non-visit activities. The listed annual total cash target pay assumes full-time employment at 40 weekly hours and full productivity, and actual compensation depends on factors such as local labor markets, education, experience, productivity, and performance.

Requirements

Current RN license specific to the state(s) assigned for work.
Current CPR certification.
At least 1 year of clinical experience as a Registered Nurse (RN).
Valid driver's license, reliable transportation, and liability insurance.

Responsibilities

Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
Makes referrals to other disciplines as indicated by the patient's needs or documents the rationale for not doing so.
Promotes patient health and independence through teaching and appropriate rehabilitative measures, including helping patients learn self-care techniques.
Supervises LPNs and HHAs.
Completes documentation timely and accurately.
Regularly communicates patient progress to the clinical manager and care team.
Plans and provides staff education.
Performs assigned on-call responsibilities and provides on-call services to patients and families.
Participates in clinical development and continuing education programs.
Performs other duties as assigned.
