About The Position

Suncrest At Brighton/Suncrest Hospice is a CHAP-certified hospice provider focused on exceptional, compassionate patient care and investing in its people. The Registered Nurse provides individualized hospice care wherever patients call home, including private residences, skilled nursing facilities, and assisted living communities.

The RN collaborates with an interdisciplinary team of nurses, Social Workers, Chaplains, CNAs, and volunteers to develop care plans addressing patients’ and families’ physical, emotional, and spiritual needs. Lower clinician caseloads and higher staff-to-patient ratios enable nurses to spend meaningful time with patients and families and provide personalized care. Various nursing opportunities are available based on individual strengths and interests.

Requirements

• Current Registered Nurse (RN) license in good standing in the state of practice.
• At least 1 year of hospice experience.
• Passion for providing exceptional, patient-centered hospice care.
• Strong collaboration and communication skills with interdisciplinary care teams.
• Ability to work independently while effectively managing a daily schedule.
• Excellent time management and organizational skills to balance patient visits, documentation, meetings, and urgent needs.
• Comfort learning and using electronic medical record (EMR) systems.
• Reliable transportation, a valid driver’s license, and current auto insurance to travel throughout the service area.

Responsibilities

• Provide compassionate, individualized hospice care in patients’ homes, skilled nursing facilities, and assisted living communities.
• Collaborate with nurses, Social Workers, Chaplains, CNAs, and volunteers on care plans that address patients’ and families’ physical, emotional, and spiritual needs.
• Manage a daily schedule and balance patient visits, documentation, meetings, and urgent needs.
• Travel throughout the service area to visit patients.
