About The Position

ChristianaCare’s Hospital Care at Home Program is seeking a full-time Registered Nurse to provide hospital-level care to medical-surgical patients in their homes. Patients remain connected with their healthcare providers through in-person home visits, virtual visits, and monitoring on a telehealth platform. The program’s care team includes physicians, nurse practitioners, physician assistants, paramedics, and registered nurses, and provides services such as laboratory testing, infusion therapy, wound care, imaging, physical and occupational therapy, EKGs, advanced practice provider home visits, virtual specialty consultations, and medication delivery.

The position is based in a hybrid setting, with work at the HCC in Newark, Delaware, and visits to patients’ homes. It is a day position with a 40-hour workweek. Weekend and holiday work follows department guidelines.

Requirements

• Active RN license in Delaware or a Compact State.
• At least three years of RN medical-surgical experience.
• BLS certification.
• Active driver’s license.
• Ability to meet the physical demands of the practice area, including standing, walking, sitting, bending, lifting, pushing, pulling, carrying, reaching, twisting, handling, stooping, and climbing.

Nice To Haves

• BSN preferred; candidates without a BSN must be committed to acquiring one within three years of hire.
• Critical care experience is a plus.

Responsibilities
