About The Position

Advocate Health is hiring a Registered Nurse for its Hospital at Home Program. The nurse delivers hospital-level acute care directly in patients’ homes, combining independent work in a home setting with the complexity of inpatient nursing and support from a collaborative interdisciplinary team. The role involves local travel within a designated service area and a manageable caseload of approximately three to four patient assignments per 12-hour shift, which may include admissions and discharges. The position is full-time, with 12-hour day shifts and rotating weekends and holidays.

Requirements

• Active RN license in the state of Illinois.
• BLS certification from the American Heart Association (AHA).
• Valid driver’s license and clean driving record.
• Strong IV, phlebotomy, and clinical assessment skills.
• Ability to work independently in home settings.
• Excellent communication and critical-thinking skills.
• Comfort with technology and remote monitoring tools.

Nice To Haves

• Two or more years of recent acute inpatient care or emergency department experience is preferred.

Responsibilities

• Deliver acute, hospital-level care in patients’ homes.
• Perform skilled clinical tasks, including IV starts, phlebotomy, and medication administration.
