About The Position

Provide skilled nursing care to patients in their homes through intermittent visits, private duty shifts, and care plan coordination. The role is described as a flexible per diem opportunity for clinicians licensed in Florida. The posting also seeks LPNs for the described work.

Requirements

• Active Florida RN or LPN license; a Compact License is accepted.
• Current CPR certification (BLS).
• Level 2 background screening compliant with AHCA requirements.
• TB screening and physical completed within the past 6 months.
• Familiarity with electronic medical records, such as Axxess.
• For RNs, knowledge of PDGM and OASIS.
• Excellent clinical judgment, communication skills, and time management.
• Reliable transportation and commitment to punctual, professional care.

Responsibilities

• Perform skilled nursing visits, including wound care, IV administration, medication management, and patient assessments.
• Provide shift-based care for patients requiring extended clinical support.
• Monitor and document patient condition and progress.
• Collaborate with physicians and interdisciplinary teams to update care plans.
• Maintain accurate records in compliance with HIPAA and CMS guidelines; RN OASIS documentation is required.
• Uphold infection control standards and agency protocols.
