About The Position

We are seeking dedicated and clinically sound RNs and LPNs to provide skilled nursing care to patients in their homes. The role includes intermittent visits, private duty shifts, and care plan coordination. It is a flexible per diem role for clinicians licensed in Florida.

Clinicians perform skilled nursing visits and shift-based care for patients needing extended clinical support, monitor and document patient condition and progress, collaborate with physicians and interdisciplinary teams to update care plans, maintain compliant records, and follow infection control standards and agency protocols. RNs are required to complete OASIS documentation.

Requirements

• Active Florida RN or LPN license; Compact License accepted.
• Current CPR certification (BLS).
• Level 2 background screening compliant with AHCA requirements.
• TB screening and physical completed within the past 6 months.
• Familiarity with EMRs, such as Axxess.
• Knowledge of PDGM and OASIS for RNs.
• Excellent clinical judgment, communication skills, and time management.
• Reliable transportation and commitment to punctual, professional care.

Responsibilities

• Perform skilled nursing visits, including wound care, IV administration, medication management, and patient assessments.
• Carry out shift-based care for patients needing extended clinical support.
• Monitor and document patient condition and progress.
• Collaborate with physicians and interdisciplinary teams to update care plans.
• Maintain accurate records in compliance with HIPAA and CMS guidelines; RNs must complete OASIS documentation.
• Uphold infection control standards and agency protocols.
