About The Position
Guardian Angel Senior Services is seeking dedicated Home Care Nurses to join our growing team. Our mission is to provide home care services with love, enhance quality of life, maintain dignity and independence, and serve our clients with honor and integrity-because after all, it is their home. As a Home Care Nurse, you will conduct client assessments, develop care plans, and collaborate with caregivers, clients, and families to ensure the highest quality of care.
Requirements
- Active Massachusetts RN License
- Minimum of one year nursing experience preferred
- Home care, hospice, case management, or community health experience is a plus
- Excellent assessment, communication, and organizational skills
- Reliable transportation and valid driver's license
- Ability to work independently while maintaining strong teamwork
Responsibilities
- Conduct initial and ongoing client assessments
- Develop, implement, and update individualized care plans
- Perform annual evals and skill assessments for Caregivers
- Monitor client health status and document care accurately
- Communicate effectively with physicians, families, caregivers, and office staff
- Provide education and support to clients and their families
- Ensure compliance with state regulations and company policies
