About The Position
Guardian Angel Senior Services is seeking an experienced and caring Home Care RN to join our growing team. This rewarding role allows you to provide personalized care and support to clients in the comfort of their own homes while promoting independence, dignity, and quality of life.
Requirements
- Current Registered Nurse (RN) license in Massachusetts
- Strong clinical assessment and communication skills
- Compassionate, dependable, and organized
- Ability to work independently and manage time effectively
- Valid driver's license and reliable transportation

- Home care experience preferred
Responsibilities
- Conduct client assessments and develop individualized care plans
- Perform annual evals and supervisions for our caregivers
- Coordinate care with caregivers, clients, families, and healthcare professionals
- Monitor client health status and document care accurately
- Educate clients and families on health management and care needs
- Ensure compliance with state regulations and company policies
- Participate in care coordination and case management activities
