About The Position

Amedisys is hiring a Registered Nurse case manager to provide home health care. The role supports patients in healing at home through assessment, care planning, coordination with the care team, patient education, and appropriate rehabilitative measures. The nurse also supervises LPNs and HHAs, documents care, communicates patient progress, provides staff education, participates in continuing education, and performs on-call duties as assigned. The position offers pay of $50.00 to $55.00 per hour, a $7,500 sign-on bonus, and mileage reimbursement.

Requirements

At least one year of clinical experience as a Registered Nurse; candidates with less than one year must be approved by the VP Clinical.
Current RN license specific to the state or states assigned.
Current CPR certification.
Valid driver's license, reliable transportation, and liability insurance.
Candidates with less than six months of clinical RN experience must participate in the RN Intern program.

Responsibilities

Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
Refer patients to other disciplines as indicated by their identified needs, or document the rationale for not doing so.
Promote patient health and independence through teaching and appropriate rehabilitative measures, including teaching self-care techniques.
Supervise LPNs and HHAs.
Complete documentation in a timely and accurate manner.
Regularly communicate patient progress to the clinical manager and care team.
Plan and provide staff education.
Perform on-call responsibilities and provide on-call services to patients and families as assigned.
Participate in clinical development and continuing education programs.
Perform other duties as assigned.
