About The Position

Empath Home Health, a member of Empath Health, is seeking a compassionate PRN Registered Nurse to provide individualized home health care to patients in Collier County, Florida, including Naples, Marco Island, Bonita Springs, Immokalee, Ave Maria, and surrounding areas. The position involves at least 10 home health visits per week and participation in required weekend and holiday on-call rotations.

Empath Home Health provides skilled nursing and therapy services to help patients recover and maintain independence at home, along with personal care support. The RN delivers patient-centered care in patients’ homes and collaborates with an interdisciplinary team to provide holistic, coordinated services. Requirements

• Graduate of an approved nursing program.
• Active Florida RN license.
• At least 1 year of direct patient care experience.
• Strong assessment, critical thinking, and patient education skills.
• Willingness to participate in weekend and holiday on-call rotations.

Nice To Haves

• BSN or MSN degree preferred.
• Prior home health RN experience strongly preferred.

Responsibilities

• Perform comprehensive patient assessments, including OASIS assessments at required intervals.
• Develop, initiate, and revise individualized plans of care as patient needs evolve.
• Deliver skilled nursing services and treatments as outlined in the plan of care.
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies.
• Collaborate with the interdisciplinary team to ensure seamless, holistic care.
• Complete at least 10 home health visits per week.
• Participate in required weekend and holiday on-call rotations.
