About The Position

All About Home Care is a Houston-based home health care agency serving Houston and surrounding communities since 2004. The agency is seeking Registered Nurses to serve patients in the Northeast area of Greater Houston. The role provides home health nursing services in accordance with each patient’s plan of care. The agency uses Kinnser software to document patient care.

Requirements

• Graduate of an approved school of professional nursing.
• Currently licensed in the state of Texas.
• Home health skilled nursing experience as an RN is required.
• High clinical skills, with emphasis on wound care, wound vac, and IV.
• OASIS experience.
• Ability to exercise initiative and independent judgment.

Responsibilities

• Provide services in accordance with the plan of care.
• Make the initial evaluation visit and regularly reevaluate the patient’s nursing needs.
• Provide services requiring substantial specialized nursing skills.
• Initiate appropriate preventative and rehabilitative nursing skills.
• Prepare clinical and progress notes for each patient visit and timely summaries of care conferences, as required by Agency policy.
• Inform personnel of changes in patients’ conditions and needs.
• Counsel patients and their families or significant others on nursing and related needs.
• Participate in and present in-service programs.
• Understand and adhere to established Agency policies and procedures.
• Process orders and notify physicians of patient needs and changes in condition.
• Complete certification and recertification orders and discharge summaries.
• Determine the amount and type of nursing needed by each patient.
• Refer patients who require specialized skills to Physical Therapists, Speech Language Pathologists, Occupational Therapists, and Medical Social Workers.
• Supervise and teach other nursing personnel.
• Conduct patient care conferences for assigned patients.
• Participate in peer review and performance improvement as assigned.
• Provide total patient care as needed.
