About The Position

Auburn Crest Primary & Palliative Care is seeking a Registered Nurse to provide care to patients managing chronic and serious illnesses in their homes. The RN carries a manageable caseload, develops relationships with patients and families, and contributes meaningfully to each patient’s plan of care. The role supports resident health, safety, quality of life, and clinical standards while providing guidance to LPNs and aides.

Requirements

• Active, unencumbered Idaho RN license.
• At least one year of nursing experience.
• Current CPR/BLS certification.
• Valid driver’s license and current auto insurance.
• Comfort working independently in a variety of home settings.
• A compassionate, steady presence when supporting patients and families.

Nice To Haves

• Home health, hospice, palliative care, or geriatric experience.

Responsibilities

• Complete thorough assessments in patients’ homes and reassess at every visit.
• Build and update each patient’s individualized plan of care with the Medical Director and interdisciplinary team.
• Coordinate care across disciplines.
• Manage symptoms, pain, medications, and wound care.
• Teach patients, families, and caregivers about the patient’s condition and care.
• Provide clinical direction to LPNs and aides, including supervisory visits.
• Support families through difficult moments with steadiness and compassion.
• Provide clinical oversight and guidance to senior living communities and LPNs, supporting quality resident care, regulatory compliance, safe medication practices, and adherence to clinical standards.
