RN Case Manager
About The Position
Elara Caring is hiring a Home Health Registered Nurse to provide one-on-one nursing care to patients in Providence, Rhode Island, and surrounding communities. The role offers the opportunity to build meaningful patient relationships and help people receive quality care at home. Nurses use clinical judgment, educate patients and families, and support patients in managing their health at home. Applicants with clinical nursing experience who are new to home health are encouraged to apply. The position includes coordinating care with a collaborative clinical team and offers free continuing education. Tuition reimbursement is available to full-time staff. Benefit eligibility may vary by position and shift status.
Requirements
- Current state license as a Registered Nurse.
- Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing.
- At least 1 year of nursing experience in a clinical care setting.
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
- Must possess a valid driver's license and maintain an acceptable driving record, or have reliable transportation to travel between patient homes and assigned work locations.
Nice To Haves
- Home health experience is preferred, but not required.
Responsibilities
- Complete initial and ongoing assessments to identify patient care needs.
- Provide skilled nursing care according to each patient’s plan of care and physician orders.
- Monitor patient conditions, report changes, and take appropriate action.
- Teach patients and families about care, safety, and managing health conditions.
- Coordinate with the care team to support safe, consistent patient care.
- Protect patient and staff privacy in accordance with HIPAA.
