About The Position
Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services. The Registered Nurse guides patients and families through the care continuum by assessing needs, coordinating care, supporting discharge and transition planning, and managing ongoing outpatient care. Under the supervision of an RN or MD, the nurse helps ensure patients receive the appropriate level of care, provides education, coaching, and treatment-decision support, and collaborates with a Care Advocate and Social Worker. The position includes patient visits and certain non-visit activities, which are compensated using per-visit-point and hourly rates, respectively. A $7,500 sign-on bonus is offered to external candidates. The role includes on-call responsibilities as assigned.
Requirements
- Current RN license specific to the state(s) of assignment.
- Current CPR certification.
- At least 1 year of clinical experience as a Registered Nurse.
- Valid driver's license, reliable transportation, and liability insurance.
Responsibilities
- Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
- Make referrals to other disciplines based on identified patient needs, or document the rationale for not making a referral.
- Promote patient health and independence through teaching and appropriate rehabilitative measures, and help patients learn self-care techniques.
- Supervise LPNs and HHAs.
- Complete documentation accurately and on time.
- Regularly communicate patient progress to the clinical manager and care team.
- Plan and provide staff education.
- Perform on-call responsibilities and provide on-call services to patients and families as assigned.
- Participate in clinical development and continuing education programs.
- Perform other duties as assigned.
