About The Position

The Registered Nurse in home health provides one-on-one skilled nursing care and treatment to patients in their homes using a comprehensive, patient-specific care plan. The role offers the opportunity to spend quality time with patients in their homes to promote health and independence, while working autonomously with support and collaboration from a full team of healthcare professionals and managing a caseload. The nurse maintains compliance with applicable laws, regulations, and agency policies, and provides accurate documentation and follow-up of patient findings to support optimal outcomes. Adaptability is essential because work schedules and daily workloads change frequently to meet patient needs. The home health nursing team works closely with office staff and physical, occupational, and speech therapy. The position serves patients across different age groups and requires acute medical-surgical experience and strong critical-thinking skills. Driving to patients' homes is required.
