About The Position

Home Health provides community health care to patients wherever they call home. Services include home health visits for adults with acute and chronic medical conditions. Within Hennepin Healthcare System’s Relationship-Based Care practice model and under general supervision, the registered nurse provides and coordinates nursing care.

Requirements

• At least 2 years of professional nursing (RN) home visiting/public health work experience.
• Valid driver’s license and access to an insured vehicle for travel to patients’ homes or agreed-upon meeting places.
• Graduation from an accredited school of nursing.
• Valid Registered Nurse license issued by the State of Minnesota.

Nice To Haves

• Public Health/Home Care professional nursing (RN) experience.
• EPIC proficiency and OASIS experience.

Responsibilities

• Protect, promote wellness, and optimize health.
• Triage patients by assessing the severity of symptoms and referring them to the appropriate setting or level of care.
• Alleviate suffering through treatment of the human response.
• Partner with patients, their families, and significant others.
• Document interventions and an individualized plan of care.
• Educate patients about their disease process.
• Assure continuity of care.
• Educate patients and families in preventative care that promotes health and well-being.
