About The Position

Transitions Hospice is hiring a hospice Registered Nurse to provide overnight clinical support across Sangamon, Cass, Morgan, Mason, Menard, and surrounding areas. Working as a member of the interdisciplinary care team, the nurse assesses and treats patients and communicates their needs with clinical confidence and compassion. The role is scheduled for four 10-hour shifts per week with rotating weekends. The pay range is $80,000–$90,000 per year.

Requirements

Active RN license in the state of employment.
Graduate of an accredited school of Nursing.
Comfort and composure working independently during overnight hours.
Strong assessment skills and sound clinical judgment in high-pressure situations.
A calm, reassuring presence for patients and families during nighttime distress.
Excellent documentation habits and communication skills.
Valid driver's license and willingness to travel locally.

Nice To Haves

Clinical experience in hospice, home health, or a related setting is preferred.

Responsibilities

Deliver professional nursing care across the full nursing process within scope of practice.
Conduct patient and family assessments and contribute to individualized care plans.
Complete visit types as needed to meet overnight patient and family needs.
Administer medications and treatments according to physician orders.
Identify nursing diagnoses from health assessment data and respond with sound clinical judgment.
Record accurate, timely clinical notes, including pain and symptom management outcomes.
Communicate changes in patient condition to physicians and obtain orders as required.
Coordinate with community health partners to support continuity of care.
Ensure equipment and necessary services are in place for patients.
Maintain and develop clinical skills through continuing education and in-service participation.
