Requirements
- Current Registered Nurse RN license in the state of practice
- 1+ year of clinical nursing experience
- Ability and willingness to travel within the branch coverage area
- Reliable transportation, valid driver’s license, and auto insurance
- Ability to lift, push, pull, or assist patients up to 50 pounds
Nice To Haves
- Associate or Bachelor’s Degree in Nursing preferred
- Home Health or Hospice experience preferred
- Experience with HomeCare HomeBase preferred
Responsibilities
- Provide skilled nursing care to patients in their homes
- Assess patient health status and identify physical, psychosocial, and environmental needs
- Develop, implement, and manage individualized plans of care
- Conduct initial evaluations and ongoing patient assessments
- Coordinate care with physicians and interdisciplinary teams
- Monitor patient progress and communicate changes in condition
- Maintain accurate and timely clinical documentation
