Requirements

Current, unrestricted Texas Registered Nurse (RN) license
Minimum one (1) year of nursing experience
Strong assessment and critical thinking skills
Valid driver's license, reliable transportation, and current auto insurance
Ability to travel throughout the assigned service area
Sit, stand, bend, lift, and move intermittently throughout the workday. Must be able to lift, push, pull, and support 50–100 pounds, including assisting with patient transfers and moving equipment.

Nice To Haves

Home Health or Hospice experience
OASIS documentation experience
Homecare Homebase (HCHB) experience
Associate or Bachelor's Degree in Nursing

Responsibilities

Performing comprehensive nursing assessments in patients' homes.
Developing, implementing, and updating individualized plans of care.
Providing skilled nursing care, treatments, and patient education.
Collaborating with physicians, therapists, and interdisciplinary team members to coordinate care.
Monitoring patient progress and communicating changes in condition.
Educating patients and caregivers to promote confidence and independence.
Maintaining timely and accurate clinical documentation.
Delivering care in accordance with regulatory and organizational standards.
