Requirements
- Current, unrestricted Texas Registered Nurse (RN) license
- Minimum one (1) year of nursing experience
- Strong assessment and critical thinking skills
- Valid driver's license, reliable transportation, and current auto insurance
- Ability to travel throughout the assigned service area
Nice To Haves
- Home Health or Hospice experience
- OASIS documentation experience
- Homecare Homebase (HCHB) experience
- Associate or Bachelor's Degree in Nursing
Responsibilities
- Performing comprehensive nursing assessments in patients' homes.
- Developing, implementing, and updating individualized plans of care.
- Providing skilled nursing care, treatments, and patient education.
- Collaborating with physicians, therapists, and interdisciplinary team members to coordinate care.
- Monitoring patient progress and communicating changes in condition.
- Educating patients and caregivers to promote confidence and independence.
- Maintaining timely and accurate clinical documentation.
- Delivering care in accordance with regulatory and organizational standards.
