About The Position

Administer nursing care for clients in their residence, coordinate care with the interdisciplinary team, clients and their families, and the referring agency. This role involves providing nursing care to patients of all ages in a clinical setting, understanding and demonstrating appropriate behavior when interacting with patients in all populations. The nurse will promote and exemplify the Company's mission, vision, and values. Essential functions include completing initial and ongoing assessments to identify the physical, psychosocial, and environmental needs of home health clients, including OASIS assessments. The nurse will regularly reevaluate client needs, evaluate care outcomes, and assist clients in recovery or maintenance of life functions, preventing illness and relapse. Developing, initiating, and revising the Plan of Care (POC) as necessary, initiating appropriate preventative and rehabilitative nursing procedures, and planning for client discharge are key responsibilities. Effective communication with clients, families, team members, and other health care professionals, including regular participation in Case Conferences, is crucial. The role requires applying infection control and universal precaution concepts, coordinating services, and informing physicians and other personnel of client changes. Monitoring assigned cases for compliance with third-party payer requirements and completing documentation in a timely manner are also essential. The nurse will supervise, teach, and provide clinical direction to other nursing personnel (LVN/LPN, HHA, CNA, etc.) and perform on-call responsibilities. Demonstrating commitment and professional growth through in-service programs and maintaining/improving competency, as well as meeting mandatory continuing education requirements, are expected.

Requirements

  • Two (2) year experience as an RN in a psychiatric clinical care setting.
  • Nursing skills as defined as generally accepted standards of practice.
  • Excellent interpersonal skills and ability to communicate effectively.
  • Demonstrates proven decision making skills.
  • If required to make visits, Hepatitis profile.
  • Must read, write and comprehend English.
  • Completion of an accredited Registered Nursing Program.
  • Current driver’s license in good standing.
  • Reliable transportation and auto liability insurance.
  • Current State professional license as an RN in any/all applicable states.
  • Current CPR.
  • On-going employment as an RN requires maintenance of a valid RN license in the state which the nurse practices in and a valid CPR certificate at all times.
  • Prolonged standing/walking required.
  • The ability to lift/transfer up to 50 lbs.
  • Ability to meet deadlines and patient needs, make quick decisions and allocate resources under stressful conditions.
  • Meet patient/family individualized psycho social needs.
  • Requires hand-eye coordination and manual dexterity.
  • Works in patient’s home in various conditions.
  • Possible exposure to blood borne pathogens, bodily fluids and infectious diseases.
  • Some exposure to unpleasant weather; PRN emergency call.
  • Ability to travel locally and work a flexible schedule.
  • A RN with a Bachelor’s degree in nursing and licensed in the state where practicing would qualify. The RN must have 1 year of recent nursing experience (recommended within the last 3 years) in an acute treatment unit in a psychiatric hospital, psychiatric home care,psychiatric partial hospitalization program or other outpatient psychiatric services.
  • A RN with a Diploma or Associate degree in nursing and licensed in the state where practicing would qualify. The RN must have 2years of recent nursing experience (recommended within the last 3 years) in an acute treatment unit in a psychiatric hospital, psychiatric home care, psychiatric partial hospitalization program or other outpatient psychiatric services.

  • Home health experience.
  • Psychiatric-Mental health Nursing Certification Preferred but not required.
  • Preferred, BSN.
  • A RN with a Master’s degree with a specialty in psychiatric or mental health nursing and licensed in the state where practicing would qualify. The RN must have nursing experience (recommended within the last 3 years) in an acute treatment unit in a psychiatric hospital, psychiatric home care, psychiatric partial hospitalization program or other outpatient psychiatric services.

Responsibilities

  • Administer nursing care for clients in their residence.
  • Coordinate care with the interdisciplinary team, clients and their families, and the referring agency.
  • Complete initial and ongoing assessments to identify the physical, psychosocial, and environmental needs of home health clients.
  • Complete assessments at appropriate time points, including OASIS.
  • Regularly reevaluate the client’s nursing needs and evaluate the outcomes of care.
  • Assist clients in the recovery or maintenance of life functions; prevent illness and relapse.
  • Develop, initiate, and revise the Plan of Care (POC) as necessary to ensure quality and continuity of care.
  • Initiate appropriate preventative and rehabilitative nursing procedures.
  • Plan for discharge of the client from services.
  • Provide effective communication to clients, their families, team members, and other health care professionals.
  • Regular participation in Case Conferences.
  • Apply the concepts of infection control and universal precaution in coordinating and performing client care activities to protect both clients and staff (OSHA).
  • Coordinate services and inform the physician and other personnel of changes in the client’s condition and needs.
  • Monitor assigned cases to ensure compliance with the requirements of third-party payers.
  • Complete appropriate documentation in a timely manner to assure compliance with company policy.
  • Supervise, teach, and provide clinical direction to other nursing personnel (ex. LVN/LPN, HHA, CNA, etc).
  • Perform on-call responsibilities and provide on-call services to clients and their families as assigned.
  • Demonstrate commitment and professional growth by participating in in-service programs and maintaining/improving competency.
  • Meet mandatory continuing education requirements of the agency/licensing board.
