About The Position

This role is for a Registered Nurse (RN) Case Manager in the St. Paul territory, serving St. Paul and surrounding communities within a 35-mile radius. The position involves practicing at the top of one's license, serving as the clinical leader for patients, and making independent clinical decisions. The nurse will manage a home health patient caseload, complete OASIS assessments, develop individualized care plans, perform skilled nursing visits, coordinate care with other healthcare professionals, provide medication management and education, monitor patient progress, and complete documentation using Homecare Homebase (HCHB). The goal is to help patients remain safely at home while reducing avoidable hospitalizations.

Requirements

  • Active Minnesota Registered Nurse (RN) license
  • Three or more years of RN experience
  • Excellent clinical assessment and critical thinking skills
  • Strong communication and leadership abilities
  • Comfortable managing an independent schedule
  • Reliable transportation for travel throughout the St. Paul territory

Responsibilities

  • Manage a home health patient caseload throughout the St. Paul service area
  • Complete OASIS assessments and develop individualized care plans
  • Perform skilled nursing visits and comprehensive clinical evaluations
  • Coordinate care with physicians, therapists, social workers, and other healthcare professionals
  • Provide medication management, disease education, and ongoing patient support
  • Monitor patient progress and adjust care plans as needed
