About The Position

This position is designed for a Registered Nurse (RN) Case Manager in Home Health, focusing on a dedicated primary territory to minimize travel time and maximize patient care. The role allows nurses to build lasting relationships with patients and communities within the western Twin Cities area. Weekend and on-call visits are kept within a manageable distance from home, and the position offers independence in the field with strong clinical support.

Requirements

• Active Minnesota Registered Nurse (RN) license.
• 3+ years of RN experience.
• Strong assessment, communication, and organizational skills.
• Ability to work independently while managing a field-based schedule.
• Reliable transportation.

• Home health experience preferred.

Responsibilities

• Manage a home health caseload throughout your assigned territory.
• Complete OASIS assessments and individualized care plans.
• Deliver skilled nursing care and patient education.
• Collaborate with physicians and interdisciplinary team members.
• Help patients remain safe, healthy, and independent at home.
• Maintain timely and accurate clinical documentation.
