You will manage a home health caseload, complete OASIS assessments and individualized plans of care, provide skilled nursing visits, assessments, and patient education, coordinate care with physicians and the interdisciplinary team, help patients remain safe and independent in their homes, and maintain accurate and timely clinical documentation.

Requirements

  • Active Minnesota Registered Nurse (RN) license.
  • 3+ years of RN experience.
  • Strong clinical judgment and ability to work independently.
  • Excellent organization and communication skills.
  • Reliable transportation for travel throughout the territory.

  • Home health experience preferred.

Responsibilities

  • Manage a home health caseload throughout Woodbury, Cottage Grove, Hastings, St. Paul Park, and Landfall.
  • Complete OASIS assessments and individualized plans of care.
  • Provide skilled nursing visits, assessments, and patient education.
  • Coordinate care with physicians and the interdisciplinary team.
  • Help patients remain safe and independent in their homes.
  • Maintain accurate and timely clinical documentation.
