RN Case Manager – In-Home Patient Care | $6,000 Sign-On Bonus
About The Position
Home health offers a different path for nurses who are burned out from long shifts, high patient ratios, and constant pressure in hospitals and clinics. In this role, you start your day from home, care for one patient at a time, and have the opportunity to think critically, build meaningful relationships, and provide the kind of care that inspired you to become a nurse. You will manage a home health caseload, complete OASIS assessments and individualized plans of care, provide skilled nursing visits, assessments, and patient education, coordinate care with physicians and the interdisciplinary team, help patients remain safe and independent in their homes, and maintain accurate and timely clinical documentation.
Requirements
- Active Minnesota Registered Nurse (RN) license.
- 3+ years of RN experience.
- Strong clinical judgment and ability to work independently.
- Excellent organization and communication skills.
- Reliable transportation for travel throughout the territory.

- Home health experience preferred.
Responsibilities
- Manage a home health caseload throughout Woodbury, Cottage Grove, Hastings, St. Paul Park, and Landfall.
- Complete OASIS assessments and individualized plans of care.
- Provide skilled nursing visits, assessments, and patient education.
- Coordinate care with physicians and the interdisciplinary team.
- Help patients remain safe and independent in their homes.
- Maintain accurate and timely clinical documentation.
