About The Position

Home Health Care, Inc. is seeking a dedicated Registered Nurse (RN) Case Manager to manage a home health caseload within a dedicated primary territory in the western Twin Cities. This role is designed to minimize driving time, allowing nurses to focus more on patient care and build lasting relationships within the community. The position offers independence in the field with strong clinical support, manageable caseloads, and a culture focused on supporting clinicians. The company has been serving patients for over 30 years and is locally owned.

Requirements

• Active Minnesota Registered Nurse (RN) license.
• 3+ years of RN experience.
• Strong assessment, communication, and organizational skills.
• Ability to work independently while managing a field-based schedule.
• Reliable transportation.

Nice To Haves

• Home health experience preferred.

Responsibilities

• Manage a home health caseload throughout your assigned territory.
• Complete OASIS assessments and individualized care plans.
• Deliver skilled nursing care and patient education.
• Collaborate with physicians and interdisciplinary team members.
• Help patients remain safe, healthy, and independent at home.
• Maintain timely and accurate clinical documentation.
