ChristianaCare HomeHealth is seeking a compassionate and skilled Registered Nurse (RN) to join our dedicated team caring for primarily geriatric patients throughout the community. If you are passionate about building meaningful relationships with patients, practicing autonomously, and delivering high-quality, patient-centered care, we'd love to talk with you. As a HomeHealth RN, you'll have the opportunity to spend quality time with your patients, support them through their healthcare journey, and help them maintain independence while receiving care at home.
Requirements
- Current RN licensure in Delaware or eligibility through a Compact State License
- Minimum of one year of RN experience
- Current valid driver's license and reliable transportation
Nice To Haves
- BSN preferred; ADN candidates may be considered with a commitment to obtaining a BSN within the designated timeframe
- Home Health, Medical-Surgical, Geriatric, or Community Health experience preferred
- IV therapy and wound care experience preferred or willingness to complete training
Responsibilities
- Provide skilled nursing care and prescribed treatments to predominantly geriatric patients in their homes
- Perform comprehensive patient assessments and ongoing evaluations
- Develop, implement, and update individualized care plans
- Administer medications, treatments, and IV therapy as ordered
- Educate patients and caregivers regarding disease management, medications, safety, and wellness
- Coordinate care with physicians, therapists, social workers, and other healthcare professionals
- Facilitate discharge planning and transitions of care
- Maintain accurate and timely clinical documentation
