Comfort Home Care is seeking a Registered Nurse (RN) for a full-time opening in the Melrose/Malden area, working a morning weekday shift. Requirements
- Active Registered Nurse (RN) license in Massachusetts.
- One year of nursing experience (home health or psych preferred).
- A drive to provide compassionate, patient-centered care.
- Reliable transportation for patient visits.
Responsibilities
- Assess and Adapt: Evaluate patient's current condition, medical needs, and overall well-being during each visit.
- Create and Modify Care Plans: Actively shape and adjust patient treatment plans based on their needs and changing conditions.
- Teach and Support: Provide medication guidance to psychiatric patients and wound care education to post-op patients to help them remain independent at home.
- Communicate and Coordinate: Inform providers, families, and care teams about patient changes to ensure timely and appropriate care.
- Document and Reflect: Submit visit notes accurately from home for compliance and continuity of care.
