Comfort Home Care is seeking a full-time Registered Nurse (RN) for the Melrose/Malden area, working a morning weekday shift. Requirements
- Active Registered Nurse (RN) license in Massachusetts.
- One year of nursing experience (home health or psych preferred).
- A drive to provide compassionate, patient-centered care.
- Reliable transportation for patient visits.
Responsibilities
- Assess and evaluate patient's current condition, medical needs, and overall well-being during each visit.
- Create and modify patient care plans based on individual needs and changing conditions.
- Teach and support patients, including medication guidance for psychiatric patients and wound care for post-op patients.
- Communicate and coordinate with providers, families, and care teams regarding patient status changes.
- Document patient visit notes accurately from home for compliance and continuity of care.
