About The Position

The Registered Nurse is responsible and accountable to the Chief Nursing Officer for the organization, administration, direction, supervision, and provision of resident care delivery and practice, in accordance with the standards and criteria as outlined in the Ministry of Long Term Care Fixing Long Term Care Act; the Compendium of Standards of Practice for Nurses in Ontario; and the policies and procedures of Albright Manor.

Requirements

  • RN with current registration with College of Nurses of Ontario
  • Completion of courses in veinapuncture, wound management, Conflict resolution, I.V.-therapy
  • Possess excellent interpersonal, creative problem solving, conflict resolution and leadership skills
  • Read, write and understand to follow/carry out instructions in English independently
  • Able to physically perform the duties and responsibilities assigned
  • Ability to meet the required physical demands of the job as outlined in the physical demands analysis
  • Vulnerable Sector Police Clearance (current within the past 6 months)
  • 2-Step TB Test (chest x-ray is acceptable as an alternative)

Nice To Haves

  • Previous experience in working with seniors
  • Completion of a course in geriatrics
  • Completion of courses in veinapuncture wound management, Conflict resolution, IV therapy
  • Computer skills in resident administration/charting

Responsibilities

  • Provide professional leadership to staff, including decision making, advanced assessment of resident biopsychosocial condition, problem solving, direction, delegation, and assignment of duties. This includes staff orientation, training, performance evaluation, supervision, and replacing staff on the schedule when call-ins occur.
  • Establish and maintain collegial relationships internally and externally to promote team cohesiveness and ensure positive outcomes for residents/family, prioritizing resident safety and well-being.
  • Participate in internal committees such as infection control, pharmacy and therapeutics, medical advisory, and health and safety. Manage and direct residential units in accordance with Albright Manor's policies, procedures, and standards.
  • Complete all pertinent resident charting documentation using the electronic records documentation system.
  • Complete all pertinent resident documentation related to RAI-MDS, as assigned by the RAI Coordinator and required by the Ministry of Long Term Care.
  • Maintain confidentiality of resident information and enforce its practice with staff.
  • Provide advanced assessment and/or active resident care as required by physician's orders or RPN consultation requests for residents experiencing acute episodes of illness or complex symptoms, instituting remedial health measures in accordance with the resident's health care directive.
  • Provide daily monitoring and care for residents with acute illness or special conditions beyond the scope of RPN practice, documenting on the resident's chart, consulting with the doctor as needed, and making decisions regarding hospital transfer.
  • Review resident's medical condition upon return from hospital, be responsible for family contact, and maintain contact until the resident is stabilized.
  • Ensure the safe and effective administration of medications with resident consent, including auditing charts for consent, supervising RPNs in medication administration, investigating medication errors, reviewing medication audit reports, dispensing medications from stock, and administering intramuscular or intravenous medications.
  • Be responsible for all intake requirements for new residents from admission up to the first multidisciplinary care conference, including initial assessment, setting up the resident's chart, providing direction to RPNs regarding immediate care needs, communicating with staff/departments, coordinating and participating in initial multidisciplinary care conferences, and charting as required.
  • Review and monitor the effectiveness of resident care plans developed by the RPN.
  • Coordinate and participate in multidisciplinary resident care conferences for significant changes in psychosocial status or at discretion, and document appropriately.
  • Conduct quality assurance activities and implement continuous quality improvement initiatives in consultation with the Director of Nursing, including auditing resident charts for accuracy, timeliness, completeness, clarity, and availability of information.
  • Conduct regular inspections of the resident/staff environment, equipment, and hazardous materials to ensure safe operation and storage, and investigate workplace incidents/accidents.
  • Review all orders prescribed by external/internal health care professionals for accuracy of transcription and relevancy.
  • Monitor frequency, location, and severity of infections and implement preventative measures, supervising staff accordingly.
  • Investigate all resident incidents/accidents, implement preventive measures, and complete appropriate documentation.
  • Audit wound care program for effectiveness.
  • Meet with each resident and/or their power of attorney for personal care, or substitute decision maker, within the first 60 days of admission to discuss and complete health care directives, including resuscitation wishes, and ensure this information is filed, kept up-to-date, and communicated to the health care team.
  • Maintain competency in and perform CPR at a basic cardiac life support level, in accordance with resident's health care directives.
  • Review, approve, and monitor requests for the use of restraints, collaborating with the health care team, evaluating care plans, consulting with physicians, communicating with decision makers, using least restraint, developing plans for elimination of restraint use, and ongoing monitoring.
  • Be responsible for staff training, compliance, and practices related to WHMIS, Infection Control, and No Lift Policy.
  • Comply with all aspects of the Manor's Health and Safety programs, including WHMIS, Infection Control, Waste Management, First Aid, Safe Handling of Residents, Early Return to Work, and specific Safe Operating Practices for the Nursing/Resident Care Department. Report unsafe conditions, accidents/illnesses, cooperate in health care treatments, and implement the Manor's fire safety plan procedures.
