About The Position

The Registered Nurse (RN) is responsible for providing professional nursing care to residents in accordance with the standards established by the College of Nurses of Ontario (CNO), the Fixing Long-Term Care Act, organizational policies, and evidence-based best practices. Acting as the Nurse in Charge during assigned shifts, the RN provides clinical leadership, supervises Registered Practical Nurses (RPNs) and Personal Support Workers (PSWs), coordinates resident care services, and ensures the safe and effective operation of the home. The RN is responsible for resident assessments, medication administration, clinical documentation in PointClickCare (PCC), risk management, incident follow-up, infection prevention and control, and emergency response activities.

Requirements

  • Current Certificate of Registration in good standing with the College of Nurses of Ontario (CNO).
  • Current CPR Certification.
  • Minimum one (1) year nursing experience.
  • Demonstrated leadership and supervisory abilities.
  • Strong clinical assessment and critical thinking skills.
  • Excellent written and verbal communication skills.
  • Proficiency in PointClickCare (PCC) or other electronic health record systems.
  • Ability to work independently and effectively manage multiple priorities.

Nice To Haves

  • Previous Long-Term Care experience.
  • Experience as Charge Nurse or Nurse in Charge.
  • Experience with Ministry inspections and regulatory compliance.
  • Knowledge of PCC Risk Management modules.
  • Additional education in Gerontology, Wound Care, Palliative Care, Dementia Care, or Infection Prevention and Control.

Responsibilities

  • Perform comprehensive nursing assessments and ongoing evaluation of residents.
  • Monitor and respond to changes in resident condition.
  • Develop, implement, and evaluate individualized plans of care.
  • Collaborate with physicians, nurse practitioners, pharmacists, and interdisciplinary team members.
  • Ensure timely follow-up of abnormal findings and clinical concerns.
  • Participate in admissions, transfers, discharges, and care conferences.
  • Provide palliative and end-of-life care as appropriate.
  • Administer medications and treatments safely and accurately.
  • Monitor residents for therapeutic and adverse medication effects.
  • Ensure compliance with medication administration policies and professional standards.
  • Investigate and report medication incidents and near misses.
  • Complete narcotic and controlled substance counts and follow up on discrepancies.
  • Liaise with pharmacy services regarding medication-related concerns.
  • Maintain accurate, complete, and timely documentation within PointClickCare (PCC).
  • Complete nursing notes, assessments, care plans, and clinical monitoring records.
  • Utilize PCC modules and clinical pathways effectively.
  • Monitor outstanding assessments and documentation requirements.
  • Ensure documentation meets regulatory, legal, and organizational requirements.
  • Identify and mitigate risks affecting resident safety.
  • Complete and follow up on incidents including: Falls, Head injuries, Skin tears and wounds, Medication incidents, Resident-to-resident altercations, Responsive behaviours, Missing resident/elopement concerns, Infection prevention and control issues.
  • Utilize PCC Risk Management tools and monitoring pathways.
  • Participate in investigations and corrective action planning.
  • Report incidents according to organizational and legislative requirements.
  • Act as the Nurse in Charge during assigned shifts.
  • Provide leadership and clinical guidance to RPNs and PSWs.
  • Assign and coordinate resident care assignments.
  • Support staff in clinical decision-making and problem-solving.
  • Monitor quality of care delivery and staff performance.
  • Promote teamwork, professionalism, and resident-centered care.
  • Assist with orientation and mentoring of new staff members.
  • Serve as the clinical leader during evenings, nights, weekends, and holidays.
  • Manage urgent resident care issues and clinical emergencies.
  • Coordinate staffing resources and respond to staffing shortages.
  • Communicate with on-call management, physicians, and external healthcare providers.
  • Address resident and family concerns as they arise.
  • Ensure continuity of operations and resident safety.
  • Adhere to infection prevention and control standards and practices.
  • Monitor residents for signs and symptoms of infectious illness.
  • Implement isolation precautions as required.
  • Support outbreak management activities.
  • Promote compliance with Public Health and organizational requirements.
  • Educate staff regarding infection prevention measures.
  • Participate in quality improvement initiatives and clinical audits.
  • Support compliance with Ministry inspections and accreditation standards.
  • Ensure care practices align with: College of Nurses of Ontario (CNO) Standards, Fixing Long-Term Care Act, Occupational Health and Safety Act, PHIPA, Organizational policies and procedures.
  • Maintain resident confidentiality and privacy.
  • Establish effective working relationships with residents, families, staff, physicians, and community partners.
  • Communicate clinical concerns promptly and professionally.
  • Participate in interdisciplinary team meetings and care conferences.
  • Provide resident and family education as appropriate.
