Requirements

  • Valid and current NYS Registered Nurse license is required
  • Two years recent clinical nursing experience with strong clinical skills including one year recent Med-Surg. experience required
  • Complies with accepted nursing professional standards and practices
  • Commitment to the Hospice Philosophy of Care
  • Ability to relate to a diverse population and workforce in a personable and effective manner
  • Able to communicate effectively, while sustaining a caring, kind and professional disposition
  • Must have excellent written and verbal communication skills
  • Proficient in MS Office Suite, EMRs and ability to learn new software programs
  • Must have daily access to a vehicle, valid driver's license/acceptable driving record and auto insurance to travel to patient locations
  • Flexible schedule with weekend rotation as needed

Nice To Haves

  • BSN preferred
  • Commits to take and pass the hospice and palliative care nursing certification exam (CHPN) within three years of hire if not currently certified
  • Hospice admission and end-of-life care experience preferred

Responsibilities

  • Provides direct care to patients utilizing accepted standards of nursing practice and consistent with agency policies and procedures
  • Administers medications and treatments according to established policy and procedure and documents all interventions for accuracy
  • Performs thorough and accurate resident assessments and provides appropriate follow-up and documentation of all findings and interventions
  • Documents assessments, interventions, resident responses to treatments, and all communications/conferences/IDT meeting, care plans and discharges
  • Conducts the admission process for all new residents
  • Generates the initial plan of care and performs ongoing assessments and care plan updates in collaboration with the Interdisciplinary Team
  • Performs ongoing symptom assessments of resident and family needs and institutes the appropriate interventions
  • Documents in the electronic medical records system accurately; completes the entries timely to reflect the patient's condition consistent with the plan of care
  • Collaborates and coordinates effectively with clinical and support staff to conduct the daily care of the resident
  • Maintains proficiency in managing intravenous access devices including intravenous medication administration
  • Informs the physician and other staff of changes in patient needs, condition, and outcome of interventions
  • Participates in hospice-sponsored in-service training as required
  • Accepts other responsibilities as assigned.
