About The Position

Make a difference when it matters most. We are seeking a compassionate, skilled Registered Nurse (RN) Care Manager to provide exceptional hospice and palliative care to patients and families throughout Licking County, Ohio. This is an opportunity to build meaningful relationships with patients and families while providing expert clinical care during one of the most important stages of life. As an RN Care Manager, you’ll work alongside physicians, social workers, aides, chaplains, and other members of an interdisciplinary hospice team to ensure every patient receives personalized, compassionate care.

Requirements

  • Graduate of an accredited nursing program.
  • Current Registered Nurse (RN) license in the State of Ohio, in good standing.
  • Current CPR certification.
  • Basic computer skills and ability to accurately document patient care.
  • Valid driver’s license and safe driving record.
  • Reliable transportation and ability to travel throughout the service area.
  • Ability to safely drive in varying weather and road conditions.
  • Ability to meet state-required automobile insurance requirements.
  • Successful completion of a criminal background check and 10-panel drug screening.

Nice To Haves

  • Two years of acute care nursing experience preferred.
  • Hospice, palliative care, home health, or community nursing experience is a plus.
  • Certified Hospice and Palliative Nurse (CHPN) certification preferred.

Responsibilities

  • Coordinate and deliver high-quality hospice care using the nursing process from initial assessment through ongoing evaluation.
  • Assess patients and develop individualized plans of care based on their clinical needs and goals.
  • Provide skilled nursing care to hospice patients in homes and care facilities.
  • Monitor changes in patient condition and adjust care plans as needed.
  • Manage symptoms and support comfort, dignity, and quality of life.
  • Educate and support patients, families, and caregivers throughout the hospice journey.
  • Collaborate with physicians and the interdisciplinary care team to coordinate comprehensive patient care.
  • Communicate changes in patient status and advocate for patient and family needs.
  • Accurately document assessments, interventions, medications, and patient outcomes.
  • Follow professional nursing standards, hospice regulations, and organizational policies.
