This full-time Registered Nurse position is responsible for the daily management of an assigned patient caseload in the Waxhaw/Southwest Union County region. The role involves ensuring timely care and documentation to meet patient needs as prescribed in the plan of care and per physician orders. The nurse will collect data from patient assessments, telephonic contacts, and EMR, analyzing it to determine issues and available resources for resolution. A key aspect of the role is identifying patients at high risk for re-hospitalization, falls, or high resource utilization, requiring intervention and case conferencing to mitigate risks and ensure high-quality outcomes. The position also involves prioritizing multi-issue situations based on criticality, patient welfare, safety, and service impact, and identifying/addressing barriers to resolution. The nurse will develop and oversee a plan of care, communicate it effectively, and direct the care coordination of a multidisciplinary clinical team, including supervising LPNs and HHAs. Performing ordered treatments and evaluating patient progress towards goals are also essential functions. Some after-hours back-up call on a rotation is required. The home office is in Monroe.
