About The Position

Partners In Home Care (PIHC), Western Montana’s leading non-profit hospice provider since 1987, is seeking a compassionate and skilled Registered Nurse to join our Hospice team. In this role, you’ll deliver high-quality nursing care and coordinate home-based services for assigned patients using an interdisciplinary, patient-centered approach. As a Hospice Registered Nurse (RN), you deliver compassionate and expert Hospice Care to patients and their families during a pivotal time. You focus on enhancing the quality of life, providing comfort, and upholding dignity for individuals facing life-limiting illnesses. This role offers a unique opportunity to make a profound impact, supporting patients through End-of-Life Care and offering essential guidance and solace to their loved ones. You serve as a vital link in a supportive care network, ensuring every patient experiences comfort and peace.

Requirements

  • Holds a current and unrestricted Registered Nurse (RN) license in the state of Montana.
  • Possesses an Associate's Degree in Nursing (ADN) or a Bachelor of Science in Nursing (BSN) from an accredited program.
  • Demonstrates at least one year of experience in acute care, home health, hospice, or palliative care settings.
  • Applies experience in providing direct patient care, including End-of-Life Care.
  • Possesses in-depth knowledge of Hospice Care principles and practices.
  • Applies expertise in Pain Management and Symptom Management techniques.
  • Excels in Patient Assessment and critical thinking.
  • Proficiently performs Medication Administration and understands pharmacological effects.
  • Understands and provides effective Bereavement Support.
  • Offers strong Family Support through empathetic communication and education.
  • Engages in effective Interdisciplinary Team Collaboration.
  • Communicates clearly and compassionately, both verbally and in writing.
  • Demonstrates strong organizational skills and the ability to manage a caseload independently.
  • Ability to stand, walk, bend, lift, and transfer patients as needed.
  • Works in various home environments, which may include exposure to different environmental conditions.
  • Possesses reliable transportation and maintains a valid driver's license for travel to patient homes.
  • Manages emotional demands associated with providing End-of-Life Care.

Nice To Haves

  • Holds a Certified Hospice and Palliative Nurse (CHPN) certification.
  • Possesses two or more years of experience specifically in hospice nursing.

Responsibilities

  • Provides comprehensive Hospice Care, focusing on comfort, dignity, and quality of life for patients.
  • Conducts thorough Patient Assessment to identify physical, emotional, and spiritual needs.
  • Develops and implements individualized care plans that address specific patient goals and preferences.
  • Manages and alleviates symptoms through effective Pain Management and Symptom Management strategies.
  • Administers medications accurately and safely, adhering to all protocols for Medication Administration.
  • Offers compassionate Bereavement Support and practical guidance to families before and after a patient's passing.
  • Provides essential Family Support, educating them on patient care, comfort measures, and the dying process.
  • Collaborates effectively with physicians, social workers, spiritual counselors, and other healthcare professionals as part of an Interdisciplinary Team Collaboration.
  • Educates patients and families on disease progression, treatment options, and available resources.
  • Maintains accurate and timely patient records, documenting all assessments, interventions, and outcomes.
