About The Position

Silverado Hospice is hiring a compassionate, skilled Registered Nurse to perform hospice admissions throughout Los Angeles County. The nurse conducts comprehensive evaluations, initiates care plans, and helps ensure a smooth transition into hospice care. The role provides dignified, patient-centered care and supports patients and families during critical moments. The position follows a Wednesday through Sunday schedule, 1:00 p.m. to 9:30 p.m., and may require work outside regular hours as needed.

Requirements

Hold a valid state Registered Nurse license.
Have at least one year of nursing experience.
Be able to lift up to 50 pounds.
Clear criminal background check, physical examination, and drug screening; employment offers are also contingent on TB testing if applicable.
Have a valid driver’s license, a good driving record, and reliable transportation.
Be willing to drive to patient locations and work outside regular hours as needed.

Nice To Haves

Hospice experience preferred.

Responsibilities

Perform comprehensive nursing evaluations to admit patients into hospice.
Assess patients for signs and symptoms of discomfort, including pain, and take prompt, appropriate action.
Initiate care plans and provide appropriate nursing follow-up after hours.
Instruct caregivers on managing patient needs and submit documentation within 24 hours of the visit.
Coordinate services for patients and families across settings to ensure quality and appropriate care.
Follow approved plans of care, modify them as needed, and communicate with physicians when appropriate.
