About The Position

Moments Hospice is seeking an Admissions Registered Nurse to help make patients’ final days, weeks, and months as comfortable as possible. The role is suited to nurses seeking autonomy and variety in hospice care. The Admissions RN participates in the admissions process, evaluates patients for hospice appropriateness, and educates patients and families about hospice and palliative care. The position is full-time, with hours Monday through Friday from 12:00 p.m. to 8:00 p.m. Work is on the road.

Requirements

• Valid Registered Nurse (RN) license.
• Currently licensed as an RN in the state where duties are performed.
• Driver’s license, insurance, and a vehicle.
• Utmost compassion and integrity.
• Previous hospital, physician practice, long-term care (LTC), or assisted living facility (ALF) experience.
• Strong presentation skills.

Nice To Haves

• 1–2 years of nursing experience (preferred).
• Previous sales or service experience (preferred).

Responsibilities

• Participate in information gathering, analysis, education, and form completion as part of the admissions process.
• Evaluate patients for hospice appropriateness, including reviewing medical records as requested.
• Obtain agreement for admission from referred patients and families who meet admission criteria.
• Analyze collected data to facilitate or obtain admission orders.
• Build effective relationships with referral sources to support the quality and responsiveness expected by Moments Hospice.
• Maintain clinical and professional competence.
• Develop effective internal relationships with appropriate admissions and patient care resource persons.
• Use interpersonal skills to educate patients and families and address questions about hospice and palliative care.
