About The Position

The Transitional Care Management Nurse utilizes a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet Medicare patients’ health needs through communication and targeted intervention to promote optimal, cost-effective outcomes in a clinic setting. The function of this position is to assist Medicare patients in the administration of their health plan benefits. The Transitional Care Management Nurse focuses on arranging or coordinating services that the patient needs to get well or stay well and remove barriers that prevent the member from engaging in an appropriate plan of care. The Transitional Care Management Nurse demonstrates strengths working independently as well as collaboratively within a multi-specialty healthcare environment. The Transitional Care Management Nurse performs interactive contact and certain non-face-to-face services with the clinic’s Medicare population that has been discharged from the hospital. The Transitional Care Management Nurse performs duties and responsibilities in a manner consistent with the Mission and Values of Hattiesburg Clinic.

Requirements

  • Graduation certificate from an accredited RN school of nursing, required
  • Current state licensure, required
  • Ability to maintain strict confidentiality and handle sensitive information with discretion
  • Ability to maintain composure when confronted with fast-paced and stressful situations/timelines
  • Ability and desire to learn new technologies
  • Must be timely and dependable
  • Must be able to multi-task, work independently and be very detail oriented
  • Must have excellent written and oral communication skills
  • Must be able to complete documentation accurately
  • Must be able to follow existing workflows
  • Knowledge of EMR, medical management and policies and procedures
  • Intermediate skills in work processing, spreadsheet and database development and maintenance

Nice To Haves

  • Three (3) years of experience, preferred
  • IV certification
  • ACLS certification
  • BLS/CPR certification, preferred
