Base salary starts at $31.94/hr . E S S E N T I A L F U N C T I O N S Collaborates with interdisciplinary team by assessing patient’s psychological, social, emotional, physical, and financial needs related to health self-management and care transitions. Assists in guiding activities, nursing treatment modalities, and interventions of other practitioners based assessed needs. Conducts marketing activities with key stakeholders, promotes and inspires referrals to the program, and educates providers on the value of the program in meeting the facility goal of decreasing unnecessary readmissions. Provides information, education, and support to patient and/or primary caregiver regarding disease(s) self-management. Builds trust and rapport with patient and/or primary caregiver, while assisting patient and/or primary caregiver to set goals for treatment and general health to develop a follow-up plan to work toward achieving their goals; provides in-patient and post-discharge visits and phone follow-up to patients. Ability to assess a patient’s condition, formulate plan of care, select appropriate interventions, evaluate patient’s response to plan of care and to explain to patient/family about this process. Assists patients in discharge plans. Other duties as assigned. E D U C A T I O N Graduation from an accredited school of nursing required. E X P E R I E N C E Previous experience in a hospital setting preferred. Minimum of three (3) years of experience in acute care nursing, home health experience or a hospital setting preferred. C R E D E N T I A L S Must have a current, valid RN license from the State of Wyoming and must be in good standing with the Board of Nursing. Must have current, valid Basic Life Support (BLS) certification or must obtain within 60 days of hire and maintain credentials throughout employment with IMH. Valid driver’s license, ability to be insured through Ivinson Memorial Hospital’s Insurance requirements. Requirements
