About The Position

This role functions as a liaison for management staff, administration, physicians, managed care companies, community organizations, and other customers. The Registered Nurse Patient Navigator will coordinate efforts for treatment and promote quality improvement and educational efforts. This role will also compile data, track outcomes, and make recommendations for process improvement. Additionally, the Registered Nurse Patient Navigator will provide referrals and support for follow-up care for patients who are uninsured or underinsured.

Requirements

Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required.
Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State.
Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire required.

Nice To Haves

BLS Provider credentialed from the American Heart Association (AHA) preferred.
