About The Position

This role involves directing patients seamlessly through every stage of treatment and follow-up care as a dedicated navigator. The Registered Nurse Patient Navigator will connect administrative staff, physicians, managed care payers, and community partners as a key central liaison. This position drives coordinated patient treatment plans while elevating ongoing clinical education and quality improvement efforts. The navigator will track outcomes and analyze key metrics to recommend targeted process and care improvements, and secure critical follow-up care referrals and essential community resources for uninsured and underinsured patients.

Requirements

Registered Nurse credentialed from the Florida Board of Nursing obtained prior to hire date or job transfer date required.
Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.

Nice To Haves

BLS Provider preferred. American Heart Association or American Red Cross accepted.
