About The Position

Provides nursing care to patients in their homes, ensuring that they receive the highest level of clinical care and support. The RN will assess patient needs, develop care plans, administer treatments, and educate patients and their families about managing health conditions at home. This role requires strong clinical skills, the ability to work independently, and excellent communication with both patients and the healthcare team.

Requirements

Graduate of an accredited school of nursing.
Current state Registered Nurse Licensure, within state of practice.
Current BLS certification.
Must have and maintain valid Driver's License.
Able to communicate effectively in English, both verbally and in writing.
Strong clinical assessment and critical thinking skills.
Active nursing license.
Ability to work independently and manage time effectively.
Excellent communication and interpersonal skills.
Compassionate and patient-focused care approach.
Knowledge of home health regulations and industry standards.
Reliable transportation to visit patients at their homes.
Mobility, sight, hearing, verbal communication, changes in temperature, exposure to disagreeable elements, exposure to biological, mechanical, electrical, and chemical hazards.
Frequent standing and walking.
Lift up to 100 pounds.
Carrying up to 40 pounds.
Pushing/pulling up to 450 pounds.

Responsibilities

Perform a head-to-toe assessment on all patients and reassessments as per policy.
Adequately assess and reassess pain, utilizing appropriate pain management techniques and educating the patient and family regarding pain management.
Revise plan of care as indicated by the patient’s response to treatment and evaluate overall plan daily for effectiveness.
Perform waived testing (point-of-care testing) per Clinical Laboratory’s and the resident care unit’s policies and procedures, and interpret results, taking appropriate action.
Perform patient care responsibilities considering needs specific to the standard of care for patient’s age.
Administer medications correctly based on age and clinical condition, following the seven (7) medication rights to reduce errors.
Formulate a teaching plan based upon identified learning needs and evaluate effectiveness of learning, including the family as appropriate.
Assist physicians with procedures and perform services requiring technical and manual skills.
Perform treatments and provide services to the level of licensure.
Treat patients and their families with respect and dignity, identifying and addressing psychosocial, cultural, ethnic and religious/spiritual needs.
Function as a liaison between administration, patients, physicians and other healthcare providers.
Interact professionally with patient/family and involve them in the formation of the plan of care.
Demonstrate knowledge of the principles of growth and development over the life span and the skills necessary to provide age-appropriate care.
Interpret data about the patient’s status to identify age-specific needs and provide care needed by the patient group.
Perform all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
Support and maintain a culture of safety and quality.
Consult other departments as appropriate to provide for an interdisciplinary approach to the patient’s needs.
Communicate appropriately and clearly to Nurse Manager, co-workers and physicians.
Maintain a good working relationship both within the department and with other departments.
Identify and address psychosocial needs of patients and family; communicate with Social Service/Discharge Planner regarding both in hospital and post discharge needs.
Ensure documentation meets current standards and policies.
Manage and operate equipment safely and correctly.
Coordinate and supervise patient care as necessary.
Demonstrate an ability to be flexible, organized and function under stressful situations.
Adhere to dress code, appearance is neat and clean.
Complete annual education requirements.
Maintain regulatory requirements, including all federal, state, local regulations and accrediting organization standards.
Maintain and ensure patient confidentiality at all times.
Report to work on time and as scheduled, complete work within designated time.
Wear identification while on duty, use computerized punch time system correctly.
Complete inservices and return in a timely fashion.
Attend annual review and department inservices, as scheduled.
Attend 12 staff meetings annually or read and return monthly staff meeting minutes.
Represent the organization in a positive and professional manner.
Actively participate in performance improvement and continuous quality improvement (CQI) activities.
Comply with all organizational policies regarding ethical business practices.
Communicate the mission, ethics and goals of the hospital, as well as the focus statement of the department.
